HC – Pharmacist – Intermediate

• Conduct comprehensive telephonic medication consultations that meet/exceed production & quality metrics.
• Complete a comprehensive medication review of prescription and non-prescription medications with emphasis on patient assessment and counseling, safe and effective use of medications, identification of potential drug interactions, and other related activities. 
• Complete pharmacotherapy assessment of patients’ claims, past medical history, and disease states.
• Assess patients for medication adherence barriers and develop a plan to maximize patients’ medication success through improving or maintaining adherence to medications.
• Deliver high quality evidence-based therapeutic recommendations to providers.
• Follow all policies and procedures related to job 

• Bachelor’s Degree in Pharmacy or PharmD
• Current and unrestricted Pharmacist license in the state where you are applying to work 
• Strong knowledge of disease state management guidelines
• Experience working independently within a cross functional team environment
• Strong patient care and provider intervention experience
• Commitment to professionalism and a dedicated desire to advance patient care
• Excellent verbal and written communication skills
• The work hours required will require flexibility to work rotating shifts between the hours of 8:00am and 8:00pm CST, Monday through Friday. 
• Extended hours and occasional weekends may be required as needed.
• Intermediate level skill in Microsoft Word, Outlook, and Excel 

Preferred Qualifications:

• Bilingual preferred with preference given to Spanish, Mandarin, Cantonese and Vietnamese 
• Additional Pharmacist License in good standing in Alabama, Kentucky, Oregon, Louisiana and Idaho.
• PharmD 2+ years 
• Pharmacy work experience in retail / community pharmacy and / or PBM, but will consider new graduate experientials
• Prior call center experience
• ACPE MTM Specialist
• Certified Geriatric Specialist
• Board Certified Pharmacist
